Jayaben Patel, APRN-CNP is a skilled Family Nurse Practitioner with over five years of nursing experience across diverse settings, including the emergency department, orthopedic/neuro unit and skilled nursing facilities. She is dedicated to patient care and utilizes evidence-based practices to provide comprehensive care. Fluent in English, Gujarati and Hindi, Jayaben is adept at connecting with a diverse patient population. Currently, at Grand Lake Primary Care at St. Marys, Jayaben provides episodic and chronic care, screenings and education, working collaboratively with her healthcare team to ensure quality outcomes. During her Master of Science in Nursing (FNP) program at the University of Toledo, Jayaben gained extensive clinical experience in primary care, urgent care, women’s health and endocrinology. She performed physical exams, diagnosed conditions and developed treatment plans for diverse populations ranging from newborns to geriatrics. Prior to becoming a nurse practitioner, Jayaben worked as a Registered Nurse at Mercy Health’s Emergency Department and Orthopedic/Neuro Unit, as well as in skilled nursing.
