Recognized by the American Diabetes Association, the Diabetes Education program within Grand Lake Health System provides education to empower clients on how to effectively manage diabetes, which can help improve clinical outcomes and quality of life.

Diabetes Education
What is the Diabetes Education Program?
- Education sessions to meet your individual needs to promote a healthier lifestyle and improve management of diabetes
- Initial consult with nurse diabetes educator/dietitian to assess individual needs and then education provided based on needs, along with setting realistic goals.
- Follow-up consults will vary depending on each individual’s needs but are important to help evaluate progress and provide reinforcement and support.
- We want to ensure that when you complete our program that you feel confident in your ability to manage your diabetes.
What are the topics covered within the program?
- Understanding what diabetes is and options for managing it
- Healthy eating, including carb counting
- Physical activity
- Monitoring glucose, including glucometers and continuous glucose monitors (CGMs), such as Dexcom or Libre
- Taking medications, including insulin and insulin pumps
- Preventing acute complications, such as low glucose
- Reducing risk for developing chronic complications from diabetes
- Healthy coping and support
- Problem-solving and goal setting
Who should receive Diabetes Education?
- Recently discharged from hospital with diabetes
- Newly diagnosed with diabetes or interested in learning more about diabetes
- Started taking or had changes in diabetes medication/insulin
- Want to learn about diabetes technologies, including: continuous glucose monitors, insulin pumps, connected ‘smart’ insulin pens, apps, etc.
- Who wants to learn about diabetes technologies, including: continuous glucose monitors, insulin pumps, connected ‘smart’ insulin pens, apps, etc.
How can I get scheduled for the program?
The program requires a physician referral. Your physician or Primary Care Provider can schedule an appointment by contacting Krissy Mullenhour RN, BSN, CDCES or JTDMH Outpatient Scheduling Department (see contact information below).
