DOT PHYSICALS

Grand Lake Occupational Health adheres to the guidelines established by the Department of Transportation. To ensure you receive the best service and timely completion of your certification card, please remember to bring the following items to your appointment:

A list of medications you are currently taking.

  • A list of all medications you are currently taking.
  • For patients with hypertension: It’s important to have a blood pressure reading within the DOT guidelines. Be sure to take your medication before your appointment.
  • For diabetic patients: Please provide documentation indicating that your condition is well-controlled, such as lab results from the past three months that include your Hemoglobin A1C and fasting blood sugar results. Additionally, bring a note from your primary physician confirming that your diabetes is well-managed.
  • For those with sleep apnea: Please bring your most recent sleep study results to your appointment.
  • If you use a CPAP machine, you’ll need to provide a printout showing compliant usage for the last three months.
  • If you are on anticoagulant therapy, bring your INR results from the past 30 days along with a statement from your primary physician indicating that your coagulation status is stable.
  • For patients with cardiovascular conditions: Please provide copies of your latest stress test and EKG, along with a statement from your cardiologist confirming that you are fit to meet the requirements for commercial driving.

Immigration Physicals

To ensure we can assist you effectively, please bring the following items to your appointment:

  • I-693 Report of Medical Examination Form
  • Photo ID
  • Copies of Vaccination Records including:
    1. Tb Skin Test (PPD)
    2. Tetanus vaccine
    3. Measles, Mumps, Rubella Immunity (MMR)
    4. Pneumococcal Vaccine
    5. Influenza Vaccine
    6. Hepatitis B Vaccine Series


Please note that the fee for the Immigration Exam is due at the time of your appointment. We accept cash, checks, and credit cards. Requirements may vary based on your age, and the physician may request additional testing, which will be billed separately and may be covered by your insurance. Please be aware that this may require one or more return visits to complete your exam.

All patients under 18 must be accompanied by a legal parent or guardian, and documentation of legal guardianship is required.

Pilot Physicals

It’s crucial that you provide our clinic with all the FAA-required information regarding your current health status. Most deferrals or denials during physical exams result from pilots not supplying this information. Additionally, please discuss any health concerns that may impact your ability to fly safely with our Aviation Medical Examiner (AME).

You are required to fill out your 8500-8 Form online. Click here to access.

To help expedite your certification process before your appointment, consider the following suggestions:

  • If you’ve experienced any changes in your vision, consult your eye doctor before your physical and bring your glasses to your appointment.
  • If you have a Special Issuance, bring the corresponding letter from the FAA. Ensure that you have submitted all relevant medical information to the FAA regarding your Special Issuance.
  • Provide our AME with any medical information requested by the FAA related to your Special Issuance.
  • Inform our AME if you have any of the fifteen disqualifying medical conditions.
  • Compile a list of all medications you are taking, including the purpose, dosage, and any side effects you may be experiencing.
  • If you have a chronic health condition, such as high blood pressure or diabetes, ensure you have regular check-ups with your family physician and have lab work done within three months before your pilot physical.
  • If applicable, bring your Statement of Demonstrated Ability (SODA).
  • Gather any additional health-related documents that you feel might be necessary, particularly if you have experienced a significant health event or change since your last certification.

Please note that the Aviation Medical Exam fee is due at the time of service. We accept cash, checks, and credit cards. Our team understands the importance of your certification, and if any issues arise, our AME will collaborate with you, the FAA, and your personal physician to find a resolution. Our goal is for you to leave our clinic with your certification in hand.
